The player's bullet power increases on a linear scale as the player collects power-ups dropped by enemies. The player can also use focus which slows down the player's movement and makes the collision box visible. Unlike another Touhou shooter, in a certain stage the player may only deal with one or two boss battle before advancing to the next level. Each game begins with three lives and the option to continue is given when all lives are lost.

Mythical Mirror features similar gameplay to Touhou Eiyashou, where the player can choose a team of two characters; Cirno-Suwako, Nitori-Momiji and Keine-Mokou, each having distinct attack types. The bombs also show different behavior and damage depending on the character. When playing with the character pairs, one character fires when unfocused, while the other switches in and fires when focused, with a reduction in movement speed.

Each character has different behaviour and different preferred items which must be memorized by player to achieve succesful trading. There are 36 characters that could be selected by the player but only 11 who have their own storyline. The spell card attack feature from the original series also plays a role in the game.

Despite being a magician, Marisa can't actually use magic in the game and only uses her broom as weapon. Between each stage players can purchase new skills, items and weapons by picking coins droppped by defeated enemies. There are several maps with various challenges and difficulties, encouraging players to repeat the missions.
