I'll be honest, I wish to know. I'm thinking of a DirectX issue, but I can't say for sure. I assume it happens even after restarting your PC? You could try using DXVK or d3d8to9 for the game and see if that fixes it.

I don't know how to do any of that but it seems to work fine after starting the game windowed instead of fullscreen

Cool! Now I know it very likely is a DirectX issue, thanks!

If you'd prefer to play Touhou in fullscreen, to use DXVK or d3d8to9, I think you just need to download them and put the d3d8.dll into the Touhou folder (where the executable is).
