extract the file if you haven't already, open the extracted file, double click on the application called "Touhou06", wait for the game to update to the latest patch, click "ok" at the bottom right(?) of the window, double click the application again, and you should be able to play it when it finishes loading and this appears (press the enter or z key to open the main menu)
