english patch has been released! and no, the game will not give you a virus. It is totally safe to play! delete your previous version of the game (if you had downloaded it previously) and redownload the game, The english patch should be called "th18 (thpatch_en).exe"

hmm, try to see if you can open the thcrap_configure.exe file in the thcrap file, and re-install the english patch. if you can, open the thcrap configure. there are instructions on how to download patches (i think you need wifi, and no they arent viruses) and download the en language pack (184) and see if you can run the english version. For the file called "th11.exe" (should be the japanese version), there will be another file called "custom.exe". Upon opening, the text will be garbled and unreadable, but the box u should look for is at the top left corner. there should be numbers in brakets, like this : (___ x ___) and simply select the resolution you want! the first option on my current version makes it fullscreen. After choosing the resolution u want, save and exit by going to the two buttons right at the bottom, and selecting the right one. A prompt with garbled text will come up, but it just means it has saved. For the english version (Touhou11.exe) , find the file called "Touhou11-Config.exe". It's all in english so impretty sure you can tell what to click. However im not sure if this works for everyone on the english patch since it always reverts to 960 x 720 for me. If u had followed the steps, the game should appear in ur desired screen size. Happy Playing!

this is a pirated version, so do not expect every feature to work. a comment above me has mentioned that you need a specific account to play online matches, but i am unsure if it works.

maybe completely delete/uninstall game files and re-download the game? i believe thpatch-en is a patch for the english translation of the game. maybe try re-downloading the patch again through the thcrap_configure.exe (located in thcrap folder)
